St. Louise Resource Services (StLRS) has successfully partnered with Health Care providers within Los Angeles County. These medical partnerships include Dr. Glaze, MD, Para Latino Medical Center, MLK Medical Group, MLK Jr. Community Hospital, Claris Health, Queens Care Health Centers, LA HealthCare, and Clinica Medica Hispana. These clinics are located within Service Planning Area 7, which focuses on low-income families, seniors, and pregnant mothers.
St. Louise Resource Services has maintained ongoing contact with these partnerships which includes monthly in-person visits, as well as constant contact through email and telephone calls. StLRS staff encourages warm hand-off support for those clients who wish to be referred immediately. Staff also ensures that these partners are stocked with fliers and information which can be provided directly to their patients.
StLRS strongly believes that these efforts have had a strong and positive outcome for community members. Within the third quarter (April – June 2022), CalFresh Outreach Assisters have serviced about 37 individuals with CalFresh applications, SAR 7 renewals, and RRRs (annual recertifications), with an 85% approval rate from completed and processed applications and renewals. Claris Health has been the highest referring clinic this last quarter. StLRS is very excited to share these outcomes and looks forward to the continuance of these relationships with the partnering medical offices. CFO staff are also actively connecting with new clinics within Los Angeles County, conducting outreach in-person and via telephone in hopes to continue connecting to community members and providing these critical services to those in need.
