Supporting Families Through Change: Helping a Single Mother Access Health Care and Food Assistance

Last year, a single mother with two children reached out to the team seeking assistance. At the time of her initial assessment, she qualified for MediCal but did not meet the income eligibility requirements for CalFresh. The Catholic Charities of Santa Clara (CCSC) team provided guidance, answered her questions, and encouraged her to stay connected should her circumstances change.
Several months later, the team reconnected with her at a community outreach event. Because of the team’s continued presence and relationshipbuilding efforts, she felt comfortable sharing an update on her situation. She explained that she was on leave from work due to an injury and was receiving Workers’ Compensation benefits, which had significantly reduced her household income.
The team conducted a new, comprehensive assessment and determined that s he was now eligible to apply for both Medi-Cal and CalFresh. They provided hands-on support throughout the process, including explaining program requirements, completing and submitting county applications, and assisting with required documentation.
Shortly thereafter, she contacted the team to share that she had been approved for both programs and had begun receiving benefits. These resources provided critical support during her recovery by ensuring continued access to health care and food assistance for her and her children. By remaining accessible and responsive to changing circumstances, the team helps ensure families receive timely, meaningful support during critical moments.
